Membership is NOT required for use of the facilities - Walk Ins welcome! An Assumption of Risk and Release and Indemnification Agreement must be signed by all participants or by a parent or guardian for those participants under the age of 18.  A photo identification card will be issued and is good for one year from date of issuance. Due to cancellations/no shows without adequate notification, we must regretfully put into place a cancellation policy.  Cancellations must be made by phone 24 hours prior to appointment time to avoid a $25.00 cancellation fee and/or the required pre-payment of next appointment. Cancellation fee is due prior to scheduling of next appointment.
